✅✅Tooth Bleaching:

⚫️Preparation:

•Case : Mucocele on lower left sided lip.

✅What is Mucocele lip❓

A mucocele of the lip is a common, harmless, fluid-filled cyst (a mucus extravasation cyst) that forms on the inside of the lip, usually the lower lip, due to trauma like lip biting, causing saliva to leak and form a soft, painless, bluish or clear bump.

✅ Sign & Symptoms:

✔️Appearance: A soft, round, dome-shaped, painless bump.
✔️ Color: Appear clear, bluish, or the same color as your lip.
✔️ Location: Most common on the inner surface of the lower lip.
✔️ Size: Can range from a few millimeters to a couple of centimeters.

✅Treatment:
Surgical removal of the cyst and associated gland.

✅A case of Melanotic Neuroectodermal Tumour of Infants(MNTI).

• Patient: A 6-months-old female infant.

• Chief Complaint: Rapidly enlarging soft-tissue swelling in the anterior maxillary region for the past 4 months.

• Onset & Course: Gradual onset but rapid progressive increase in size, causing significant parental concern.

➡️ Symptoms:
• Frequent bleeding on touch.
• Bleeding episodes while breastfeeding, leading to feeding difficulty.
• Irritability related to repeated bleeding.
• Fever.

➡️ Clinical Appearance:
• Large, expansile, soft-tissue mass on the anterior maxilla.
• Surface shows dark/necrotic/pigmented areas suggestive of melanin or hemorrhage.
• Lesion is vascular, friable, and bleeds easily.
• Surrounding mucosa stretched and erythematous.

• Duration: 2-3 months

• Age Relevance: Occurrence in an infant under 6 months strongly suggests a neuroectodermal or vascular origin.

• Provisional Diagnosis: Features highly suggestive of Melanotic Neuroectodermal Tumor of Infancy (MNTI).

✅Investigation:

• CBC with ESR
• Serum creatinine, Serum Electrolytes
• Anti-HCV, HBsAg
• Chest Xray P/A view
• Echocardiogram
• CT scan
• Blood grouping, Rh factor, Cross matching

✅Differential Diagnosis:

• Infantile hemangioma
• Congenital granular cell tumor
• Rhabdomyosarcoma
• Pigmented teratoma
• Neuroblastoma metastasis
• Vascular tumor with secondary infection (explains fever)

✅Treatment Given:

• Under General anesthesia.
• Wide surgical excision of tumor with 5mm healthy tissue margins to reduce the high risk of recurrence.
•Associated procedure: Teeth and developing teeth within the tumor area are often removed during the procedure.
• Hemostasis achieved.
• Closure was done by 3-0 vicryl & 2-0 vicryl suture.
• Specimen sent for histopathology.
• Post-operative antibiotics (due to fever and risk of infection).
• Analgesics
• Feeding guidance
• Close monitoring

✅Prognosis:
• Good after complete excision
• Recurrence rate ~10–20%
• Regular follow-up essential
