A couple of dam changes later and annoyingly some camera issues resulting in me changing from my ring flash to twin flash 😖 …. we have a couple of stress-reduced direct composite restorations and an overlay prep ready for cementation.
Biomimetic protocols followed.
(Contacts broadened to try and minimise food impactions and make cleaning easier for the patient)
Now excuse me for going quiet for a few months whilst I go and have a little rugrat 👶 🤭🤨

Prosthetically driven implant placement.
implant planning software used.

Daily direct composite
BRD protocols as taught by my fab mentors and .
1. Caries removal end points established with caries dye.
2. Air abrasion.
3. Prime and bond with SE protect.
4. Decoupling with time 5 mins.
5. Resin cost with majesty flow.
6. EverX posterior and AP-X for dentine replacement.
7. Enamel replacement with filtek.

Little changes to just one tooth for this lovely patient.
Patient wants to maintain a natural look so the others left well alone.
Bioclear matrix used to close spacing (come in a range of sizes so very helpful).
Empress Direct by Ivoclar composite used.

Nearly didn’t post this as not perfect I am fully aware!
Very important to manage patient’s expectations in these cases.
Very tricky case of replacing just one large composite filling. (history of trauma - tooth darker than adjacent teeth BUT still vital so winning 🥳)
Patient was very happy with end result because time was taken to explain what we could achieve and areas of limitation.
Now back to our Ortho team to finish up the Invisalign and remove the attachments.

Biobase for Emax Overlay.

(15 and 17 on patient’s treatment plan)
Biobase Process:
1. Caries removal end points established using caries detecting dye.
2. Air abrasion.
3. Prime and bond with SE protect.
4. Decoupling with time (5 mins)
5. Resin coat with majesty flow.
6. Ribbond place over pulp embedded in AP-X.
7. DME mesially and distally using AP-X.
8. Biobase restored with EverX Posterior followed by AP-X.
9. Glycerine cure.
10. Scan taken and sent to lab for definitive restoration.

Emax onlay fitted today using warmed AP-X.
