Take Me Home Program Fort Bend County Sheriff's Office

The Take Me Home™ Program is a voluntary and confidential registry designed to help individuals with cognitive impairments who may struggle to communicate if lost or in crisis. Developed initially by the Pensacola Police Department and now supported locally through Fort Bend County Sheriff’s Office and ImageSoft’s SafeEncounter platform, the system stores a digital photo, personal details, and caregiver contacts.

Eligibility:
Fort Bend County residents (THIS INCLUDES FULSHEAR) with cognitive impairments such as Alzheimer’s, Autism, Dementia, or Down Syndrome who may be at risk of wandering. It excludes individuals with only physical, hearing, or vision impairments or those able to communicate through interpreters.

How It Works:
If a registered individual is found by law enforcement and cannot identify themselves, officers can access the secure database from their vehicle or station to match them by name or physical traits and reunite them with loved ones. It also aids in missing person cases by providing immediate access to the individual’s profile.
