ROLES OF PHYSIOTHERAPY IN PRIMARY HEALTH CARE (PHC) IN ZAMBIA

Physiotherapy practitioners in Zambia contribute significantly to Primary Health Care (PHC) by supporting health promotion, disease prevention, early detection, and functional optimization for communities. Their role is strong in NCDs and trauma, which are two major burdens in the country.

1. PHYSIOTHERAPY IN HEALTH PROMOTION & PREVENTION OF NCDs

NCDs in Zambia—such as hypertension, diabetes, obesity, stroke, chronic respiratory diseases, and some cancers—are strongly influenced by lifestyle and physical inactivity. Physiotherapists play the following roles:

A. Community-Based Lifestyle Education

- Conduct health talks at clinics, schools, workplaces, and churches.
- Promote physical activity, weight control, smoking cessation, and reduction of sedentary behaviour.
- Educate communities on safe, culturally appropriate exercise for all age groups.

B. Physical Activity Prescription & Exercise Programmes

- Provide personalized exercise plans to prevent: Hypertension, Type 2 diabetes, Cardiovascular disease, Obesity, Muscle weakness in older adults etc
- Lead community exercise groups, walking clubs, and physiotherapy-led fitness programmes.

C. Screening & Early Detection

- Physiotherapists can screen for:
- Postural problems
- Mobility deficits
- Early signs of cardiovascular or respiratory compromise
- Early signs of Cerebral Palsy (CP) and other pediatric Physiotherapy indicated conditions to help prevent or limit disability (Eg During Child Health Week)
- Fall risks in the elderly
- Musculoskeletal conditions related to lifestyle
- Physiotherapy practitioners can then refer to appropriate services, supporting early intervention.

D. Workplace Health Promotion

- Ergonomics training to prevent musculoskeletal disorders.
- Injury prevention for mining, farming, construction and industrial workers.
- Promotion of active breaks, safe lifting, and correct workstation setup.

E. Chronic Disease Self-Management Education

- Physiotherapists help patients manage chronic NCDs by teaching:
- Home exercise routines
- Breathing techniques for asthma/COPD
- Energy conservation
- Safe physical activity for individuals with hypertension or diabetes

2. PHYSIOTHERAPY IN PREVENTION & MANAGEMENT OF TRAUMA

Trauma is a major cause of disability in Zambia (road traffic accidents, occupational injuries, falls, assaults). Physiotherapy has essential PHC roles in both prevention and post-trauma functional recovery.

A. Injury Prevention Programmes

- Community education on road safety, correct helmet use, and safe physical activity.
- Training in fall prevention for elderly and vulnerable populations.
- Workplace injury-prevention in high-risk sectors: mining, industry, agriculture.

B. First-line Functional Assessment in PHC Facilities

Physiotherapists can:
- Assess mobility after injury
- Identify patients who need urgent referral
- Provide early movement guidance to prevent complications

C. Early Rehabilitation to Prevent Disability

- Even at PHC level, physiotherapists:
- Maintain joint mobility, muscle strength, and function early after trauma.
- Provide splinting, positioning, and mobilization advice.
- Prevent secondary complications such as: Contractures, Pressure sores, Chest complications, Muscle atrophy etc.

D. Community-Based Rehabilitation (CBR)

This is vital in rural Zambia, and a number og training institutions in Zambia have incorporated CBR as part of the training programs or courses. Under this, Physiotherapists:
- Provide home/community rehabilitation for trauma survivors.
- Train caregivers and community volunteers.
- Support reintegration into school, work, and community activities.

3. CROSS-CUTTING ROLES IN PHC FOR BOTH NCDs & TRAUMA

A. Multidisciplinary Collaboration

Physiotherapists work with: Nurses, Nutritionists, Clinical officers, Doctors, Social workers, Community health assistants etc, to provide integrated care.

B. Capacity Building

- Training community health workers in basic exercises and early mobility.
- Teaching safe movement, lifting, and ergonomic principles.

C. Policy & Public Health Involvement

Physiotherapists contribute to: National NCD strategies, School health policies, Workplace safety guidelines, Public health campaigns

D. Rehabilitation for Persons with Disabilities

They ensure long-term functional independence, reducing the burden on families and health-care facilities.

MOST RISK FACTORS OF FATTY LIVER.

1. Being Overweight or Obese
Excess body fat—especially around the belly—increases fat buildup in the liver.

2. High Sugar Intake
Regular consumption of sugary drinks, pastries, white bread, and processed snacks leads to insulin resistance and liver fat storage.

3. Excess Alcohol Use
Heavy or regular drinking directly damages liver cells and causes fat accumulation.

4. Type 2 Diabetes
High blood sugar and insulin resistance make the liver store more fat.

5. High Cholesterol & High Triglycerides
Uncontrolled cholesterol levels make fat more likely to deposit in the liver.

6. Sedentary Lifestyle
Lack of movement slows metabolism and increases fat buildup.

7. Unhealthy Diet
High intake of:

Deep-fried foods

Processed foods

Red meat

Sugary snacks

Fast food

8. Belly Fat (Visceral Fat)
Even if you are not overweight, fat around the stomach raises fatty liver risk.

9. Rapid Weight Gain or Rapid Weight Loss
Both put stress on the liver.

10. Genetic Factors
Family history of fatty liver increases risk.
