PDO monothreads are often misunderstood.
Social media tends to focus on complications, migration, or dramatic results, but the reality is that monothreads are a very different treatment from lifting threads. When used in the right patient, by an experienced practitioner, they can be an excellent tool for improving skin quality and supporting collagen production.
PDO (Polydioxanone) is a biocompatible material that has been used safely in surgery for decades. Once placed beneath the skin, the threads gradually dissolve over several months. During this process, they create a controlled healing response that stimulates fibroblasts — the cells responsible for producing collagen, elastin, and extracellular matrix.
In many ways, the principle is similar to collagen stimulators such as Sculptra or Lanluma. The goal is not to add volume, but to encourage the body to produce its own structural support over time. When we talk about skin aging, we often focus on what we see.
But what changes first is invisible.
The extracellular matrix gradually loses structural hyaluronic acid.
Collagen fibers become fragmented.
Oxidative stress and glycation alter tissue behavior long before deep folds appear.
In this case, the objective was not to “add volume,” but to influence tissue quality at a molecular level.
A high molecular weight hyaluronic acid protocol (up to 2.6 MDa) was selected to provide longer dermal persistence and structural support.
The formulation also includes trehalose — a disaccharide studied for its stabilizing and antioxidant properties. Trehalose helps maintain hyaluronic acid chain integrity under oxidative stress and supports extracellular matrix balance.
