Prevalence and Onset:
It affects a large portion of men and becomes more noticeable with age:
• Roughly 30-50% of men experience it by age 50.
• By age 70-80, this rises to around 70-80% in many populations (especially Caucasian men; rates are often lower in Asian or African descent groups).
• It can start as early as the late teens or 20s but often becomes prominent in the 30s-50s. 
The classic pattern involves receding hairline (often forming an “M” shape at the temples), thinning at the crown (vertex), and eventually a horseshoe-shaped band of remaining hair around the sides and back.

Causes:
It’s primarily driven by:
• Genetics — Strong hereditary component. If close male relatives (especially father) had it, your risk is higher.
• Hormones — Dihydrotestosterone (DHT), a byproduct of testosterone, binds to sensitive hair follicles, causing them to shrink (miniaturization). Over time, hairs become finer, shorter, and eventually stop growing. 
Other factors like age, stress, nutritional deficiencies, or certain medications can accelerate or contribute, but genetics + DHT are the main drivers. It’s not caused by wearing hats, frequent shampooing, or most lifestyle habits alone.

Is It Reversible?
There’s no permanent cure, but treatments can slow, stop, or partially reverse progression if started early. Results take 6-12+ months and require ongoing use—stopping usually leads to resumed loss.

As we age, facial muscles (primarily the mimetic muscles responsible for expressions) undergo several key changes, including atrophy, loss of tone/elasticity, and sometimes increased resting tension or remodeling. These shifts interact with skin thinning, fat redistribution/loss, bone resorption, and gravity to produce visible aging signs like sagging, wrinkles, and loss of facial contour.

Look at this small point at the corner of the mouth, it may look minor but anatomically it is one of the key connection points in the lower face.

This point is called the Modulus. The Modulus is not one muscle. It is a hub where several facial muscles meet and influence the position of the mouth corner.

Here we have the Orbicularis Oris, which closes and compresses the lips the Buccinator which supports the cheek from inside, the Zygomatic Major and Levator Anguli Oris which lift the mouth corners and the Risorius
which pulls it outward when we smile.
