Nursing support for chronic, complex, and person-centered care during illness or injury within the community.

Who can this program benefit?
This model can be utilized for both people and animals effected by:
• Chronic or Complex pain
• Someone planning for surgery for an illness or injury (this must be approved by their surgeon prior to commencing).
• Someone who has recently has recovered from surgery for illness or injury that still needs support (this must be approved by their health practitioner prior to commencing).
What will be achieved:
Patients with chronic or complex pain have complex needs.
People planning for surgery or post-surgery also have complex needs that often cannot be addressing within the short consultation with their specialist or health care professional.

This will give people the opportunity to take their time to assist in development in a program that best suits their current needs. This will also give the added support that people would normally receive in a hospital inpatient admission but instead from a nurse within their usual home environment. It will also utilize supports within their current support network.

I believe this will prevent multiple Emergency admissions, multiple GP appointments, and people suffering in silence about their stress and anxiety because the 24/7 support is not available and as we know as health professionals, support is often needed when no one is around and outside of business hours.
