For patients new to lip filler, who desire plump lips that look natural, I advise my patients of 2 lip filler sessions spaced 4-6 weeks apart to build the lip volume up slowly to allow the lip tissue to expand gradually. This helps to avoid lip filler migration and the dreaded ducky 🦆 lip appearance.

As you can see, my lovely patient above has a thin upper lip compared to her lower lip, and her goal was to have fuller lips while looking natural. I focused on building up vertical height to the upper lip and hydrating the upper and lower lip.

It is important to note that most patients will require 2️⃣ sessions if it's their first time getting lip filler (or after dissolving and starting fresh) since the initial volume will go down about 20-30% and it's mostly for "lip hydration". Then during the second session most of the added volume will last longer as we add the second "layer" to fine tune the shape and volume of your lips.

Why is it important to treat the whole face, using different products?

Because in order to look balanced and natural, we must treat the different causes of aging, and this is done by addressing the different layers of the skin. 

1️⃣ Skin health - medical skin care products, morpheus8, laser, peels, microneedling, Tixel

Full face neurotoxin was done in the first session, then she returned for filler and Sculptra treatment at the second session. The second photo is taken just after her treatment, so these are not the healed results.

I wanted to share a 2 month progress photo of my lovely patient with melasma, who is undergoing the 4 month Dermamelan protocol with my aesthetician .trinh . She is half way through the program and she is pleased with the results so far, since she's been affected by melasma for decades. Not only is her pigmentation lightening up in intensity, you can see the affected area shrinking. The dermamelan treatment program not only addresses pigmentation but also results in overall anti-aging effects, resulting in improvement in skin tone & texture, and smoothing out wrinkles - giving an airbrushed appearance.

Lip filler 💋

My patient is so kind-hearted and has such great energy. I've had the pleasure of knowing her for nearly 2 years. She is also in the service industry, taking great care of clients. It was my pleasure to help her with her goal of achieving lip plumping and natural looking results.

I focused on adding volume to the top lip to balance the proportions to her lower lip. Lip filler generally requires two sessions, with the first session adding hydration back to the lip and the second session layering more volume to fine tune the shape and size. I like to build up the lip slowly to allow the lip tissue to accommodate the volume gradually to minimize the risk of filler migration and the ducky lip appearance.

We will let the lip filler settle for 3-4 weeks then reassess . There will be initial swelling and you can expect the volume to decrease about 20% as it integrates.

Masseter slimming and TMJ pain relief with neurotoxin

Jaw pain can be due to excessive clenching and grinding of the teeth. Part of the cause of dental clenching and grinding, is the strong masseter muscle which we use to chew our food. We can target the masseter muscle, by weakening it with neurotoxin. As a result, The masseter muscle shrink in size and decrease the tension on the teeth and draw - this in turn can result in a slimmer face and reduction in jaw, jaw and facial pain and headaches, respectively.

Our lovely patient had been experiencing jaw pain and excessive teeth grinding and clenching. Upon treating her masseter muscle, we were able to reduce her pain as well as slim her face to create more of a heart shape to her face. The before and after was taken 2 months apart. The facial slimming results can last up to 6 months.

I seriously have the best patients. This sweet lady came to me after her skin flared up upon moving here from her home country in Southeast Asia.

To address her acne, our team:

Lip filler is a popular request among patients because it makes them feel sexy and adds the cherry 🍒 on top on an already beautiful face. I love seeing my patients feel more beautiful and confident with these treatments.

My goal is to work with my patient's anatomy and achieve a natural enhancement, I also add volume slowly and recommend a second session to avoid the overfilled look and try to avoid filler migration which can happen by using too much volume too quickly in one session. Allowing the lips to slowly expand and accommodate the new volume is best 👏

Facial balancing with injectables

My lovely patient was looking for facial rejuvenation. During my full face assessment, I could see the common signs of aging with some temple hollowing, midcheek flattening and jowling near the chin area which results in a squaring of the facial contour.

We can address some of the signs of aging by using injectables:
1. HA dermal filler and collagen stimulator (Sculptra and Radiesse) to add volume
2. Collagen stimulator (Sculptra and Radiesse) to improve skin quality
3. Neuromodulators to relax certain muscles that add bulk to the lower face (masseter muscle)  or pull down the lower face

For this patient we used only dermal filler to add volume to the cheeks to support the undereye and give a youthful volume back to the cheeks, and we addressed the chin and chin shadow. This combination helped to go from a square-shaped face 🟥 to a more heart-shaped face ❤️

Going forward, I recommend collagen stimulators annually to improve the skin quality and help to provide support to the lateral aspect of the face to decrease the tendency of our tissues from slipping down and towards the center of the face which causes the deepening of the smile lines (nasolabial folds) and marionette lines that bother so many people.

Melasma Treatment

Melasma is a very tricky skin condition to treat. My patient had significant sun exposure prior to noticing an increase in her pigmentation. I diagnosed her with melasma and we started a combination of home skin care products in combination with office treatment, and of course, consistent sun protection and if possible strict sun avoidance. Her skin tone has significantly improved, and she continues to be diligent about minimizing sun 🌞 exposure.

What can you do to treat melasma?

1️⃣ Sometimes melasma can resolve on its own if the trigger is removed, for example if pregnancy was the trigger then sometimes melasma can resolve after having the baby; or if birth control pill caused the melasma then stopping the pill can resolve it.

2️⃣ Sun protection
🔸️Just staying out of the sun for an extended period of time can help settle the intensity of the melasma (for example, melasma can be more intense during summer time, and fade during winter time).
🔸️If you will be outdoors, try to seek the shade, avoid the peak sun hours between 10 AM to 4 PM, wear broad-spectrum sunscreen SPF 30 or higher daily and reapply every 2 to 3 hours while outdoors, and wear a wide-brimmed hat

3️⃣ Skincare Routine
🔸️As mentioned above, broad-spectrum sunscreen SPF 30 or higher is crucial.
🔸️Vitamin C works great in combination with sunscreen as an antioxidant and skin brightener
🔸️ Skin lightening agent that inhibits the production of melanin or pigment on the skin, such as hydroquinone
🔸️Vitamin A derivatives, such as Retinol and tretinoin

4️⃣ Treatments
🔸️Chemical peel
🔸️Microneedling

🔸️Transexamic acid
🔸️Platelet-rich plasma (PRP)

5️⃣ Make up
You can use foundation or tinted sunscreen that can contain titanium dioxide, zinc oxide, and iron oxide which provide a dual benefit, as they act as cosmetic concealers and sunscreen.

Can you believe these before and after pictures were taken 3 months apart?

My lovely patient has been dealing with bothersome active acne flares, acne scars, and hyperpigmentation due to acne (PIH). As you can see, the spots on her neck have completely cleared up, and her overall complexion has significantly improved, revealing a brighter skin tone and even texture.

The patient is very happy with her skin journey so far.
Our next steps is to build up her tolerance to topical tretinoin and start BHA chemical peels to help regulate her oil production, exfoliate, hydrate, and to clear her pores to reduce and prevent new acne from forming.
