I am in a state of shock, disbelief and trauma at what happened in the past 24 hours (actually not even a full 24 hours).

I left Evie last night at 8:15pm as she was fast asleep and went to sleep at Scott House (five minutes from the ward). I was sat eating a takeaway when at 9:15pm I was called to say that Evie’s Berlin Heart wasn’t filling and ejecting properly and that she was awake and upset. So I went over to comfort her, now I’ve heard of this happening before with other patients but usually something is identified and fixed such as flow changes on the VAD or extra fluids needed.

For clarification, the Berlin Heart sucks the blood in which fills the ventricle chamber through the in flow pipe and ejects the blood out via the outflow pipe. When I walked in, we had PICU and all the reg doctors around Evie trying to figure out why this was happening. She had an ECHO performed straight away which could not identify any possible cause of this. She had extra fluids which also didn’t change anything on the flows. The chamber was filling up fine but not ejecting which suggested that there was an obstruction in her outflow pipe most likely a clot or collection but they couldn’t be sure until they ran further checks.

Evie was then immediately admitted into PICU and intubated because of the danger that her own heart would deteriorate quick now that the Berlin Heart wasn’t working properly. Her heart rate was in 170s/180s as she was also very upset and scared so definitely a big issue. The risk of cardiac arrest was high with her heart unsupported but thankfully she didn’t arrest when intubated. She then went for a CT scan which confirmed that there was some obstruction between the outflow pipe and the aortic valve. She was then taken to the labs to have a stent fitted into her VAD where the obstruction was. Now this was the really serious life threatening part. A stent is designed to keep the artery open to keep the blood flow from the VAD. They insert it and then expand where the obstruction is. The huge risk was that if they expand the stent and a clot flies off as a result then she would be likely to have a stroke.

We were told with two surgeons and two doctors in a room how serious this issue was. ‘This is really bad’ the opening line to us. They had discussed two options which one was the stent, the second option would have been full open heart surgery again to try and dislodge the clot or reattach a new pipe but the risks of this were also stroke, risk of bleeding out, longer in theatre and longer recovery etc. The stent was not invasive although probably posed a bigger risk of stroke but that was the option decided because of the other risks with open heart surgery. We were told she may end up on ECMO if she doesn’t cope. We were given a very bleak picture.

Evie was in the cardiac catheter labs 2am-5am and we were told they have done everything they needed to do and managed to restore the flows on the VAD. We have had a lot of significant worry about a stroke but Evie has been incredibly stable since this procedure, her actual heart held its own well they said. As she responded so well, they extubated her this afternoon and no other breathing support is needed and is moving appropriately, talking as normal (albeit very quiet volume). Very grumpy from the sedation but alert. She also had another transfusion but her HB was already getting low prior to this. She should hopefully be back in HDU in the next day or so if she remains well. I am far too scared right now to even think about things.

I cannot even begin to say how in awe I am of the team here at the Freeman. The rapid response last night was incredible. The surgeons again, can’t thank them all enough. People were woken in the middle of the night and some coming in from home or staying longer on their shifts to ensure Evie had the best care.
