Types of Skin Lesions – Exam Oriented Golden Points (Nursing) 🔹 Macule – Flat lesion, only color change, no elevation (e.g. freckle). 🔹 Papule – Solid, raised lesion < 1 cm (seen in acne). 🔹 Nodule – Solid, raised lesion > 1 cm, deeper than papule. 🔹 Vesicle – Small, clear fluid-filled blister (e.g. chickenpox). 🔹 Pustule – Vesicle filled with pus (infection sign). 🔹 Plaque – Broad, flat-topped raised lesion (psoriasis). 🔹 Scale – Keratin flakes on skin surface (dandruff). 🔹 Fissure – Deep linear crack reaching dermis (athlete’s foot). 🔹 Ulcer – Loss of epidermis ± dermis, heals with scar. 🔹 Scar – Fibrous healed tissue after injury. 🔹 Erythema – Redness due to vasodilation, blanches on pressure. 🔹 Cyanosis – Bluish discoloration due to hypoxia. 🔹 Vitiligo – Well-defined depigmented patches, autoimmune cause. 📌 NCLEX Tip: 👉 Primary lesions = macule, papule, nodule, vesicle, pustule, plaque 👉 Secondary lesions = scale, fissure, ulcer, scar.
