This is why placement will always matter more than the amount of filler. 👏

This client came to me after having 0.5ml of filler elsewhere. She wasn’t happy with the result, so a couple of weeks ago we dissolved her lips and started with a clean canvas.

Today, we replaced the same amount of filler… another 0.5ml.

So why do they look completely different?

Because this wasn’t about adding more product. It was about understanding her anatomy.

She naturally has M-shaped lips, which are one of the more challenging lip shapes to treat. A standard lip technique will often just make the existing shape bigger, rather than improving the balance.

Before we even talked about filler, we talked about her goals.

✨ She wanted her lips to stay natural.
✨ She wanted the sides to look fuller.
✨ She wanted less of her lip to disappear when she smiled.
✨ She wanted to keep a soft, rounded Cupid’s bow.

That conversation dictated every injection point.

Using Russian fanning, I focused on opening the lateral tissue and creating support where she actually needed it, while avoiding unnecessary weight through the central tubercle. The result is a straighter vermilion border, better balance and a lip that suits her, rather than a one size fits all technique.

This is why no two sets of lips should ever be treated the same.

This result is the outcome of 4 treatments over the past 2 years. 🤍

We’ve taken a slow, staged approach, focusing on improving vermilion height, maintaining a crisp vermilion border, and creating beautiful balance rather than simply adding volume.

Using strategic vectoring, I’ve directed the filler to create lift through the body of the lip, encouraging vertical height and tissue support rather than excessive forward projection. I’ve deliberately avoided overfilling the lateral lip and oral commissures, which helps preserve a refined shape and prevents that heavy, “sausage lip” appearance.

Every appointment has been about respecting her anatomy, refining the proportions, and building the lips gradually, proof that great lips aren’t created in one session, they’re created with a plan. ✨

When people see a before & after, they often think it’s just a case of putting filler where the line, or heaviness is.

But facial rejuvenation is so much more than that.

This lovely client actually used to travel all the way to East Grinstead because she couldn’t find anyone locally offering an 8 point facelift approach. Thankfully, she found me. 🤍

For her bespoke treatment plan, we restored support through the mid cheek and lateral cheek before placing filler into the piriform fossa to replace the deep structural support that’s naturally lost over time.

We then used a very small amount in the nasolabial fold itself.

Notice I said a very small amount.

Because we’re not trying to erase every line. Nasolabial folds are completely normal, and overfilling them is often what creates an unnatural result.

To support the lower face, we restored structure around the mandibular angle, giving the tissues something stronger to anchor to and helping lift the skin back into position. We also treated the pre jowl area to create a smoother, more supported jawline.

When we’d finished, I could still see a tiny amount of heaviness.

Could I have added more filler?

Yes.

Should I have?

No.

Her face was already beautifully balanced, and adding more filler would’ve changed her proportions rather than improved them.

Instead, once everything had settled, we chose Profhilo Structura to address that final bit of heaviness without altering the shape of her face.

She’s only had her first session… and these are her results already. ✨

This is why I’ll always say that the best results come from treating the face as a whole not chasing one line, fold or shadow.

Sometimes filler is the answer.
