Facial aging does not happen in just one way 👇

In aesthetic medicine, we often see four major aging patterns. Some faces age through gravitational descent, where tissues gradually shift downward, the jawline softens, and jowls begin to form. Some age through fine wrinkling, where the skin becomes thinner, drier, and more lined over time. Others follow a more muscular pattern, where strong facial movement gradually etches deeper expression lines into the forehead, glabella, and around the eyes. And some faces age through volume loss, where the cheeks flatten, the under-eyes hollow, and the face starts to look more tired or deflated.

What makes these types different is not just how they look — but which aging mechanism becomes dominant first. In one person, the biggest change may be skin thinning. In another, it may be fat pad descent, muscle activity, or loss of structural fullness.

What they do not mean is that every face fits perfectly into only one box. Most people have a mixed pattern. A face can have lower-face heaviness and fine wrinkling. Or volume loss and strong expression lines. These categories are best understood as the main direction a face tends to age — not a strict label.

Some patterns are also seen more often in certain facial structures. Deformation aging is more common in faces with thicker skin, heavier soft tissue, and more fullness in the mid and lower face. Fine wrinkled aging is more often seen in thinner, drier skin with less subcutaneous fat. Muscular aging is more common in faces with strong facial movement and expressive muscle activity. Atrophic or tired aging is often seen in narrower faces with less natural midface volume and earlier hollowing under the eyes.

That is why two people can be the same age — and age in completely different ways.
