A gentleman visited the clinic with the chief complaint of spacing between his teeth and expressed a desire to undergo alignment. He reported a previous history of orthodontic treatment with aligners, followed by relapse.

The patient was keen on pursuing orthodontic correction again; however, clinical and radiographic evaluation revealed generalized bone loss, making orthodontic treatment an unfavorable option. Therefore, restorative treatment options were discussed to address the patient’s aesthetic concerns.

The proposed treatment plan included:

Veneers for the maxillary central incisors and the mandibular right lateral incisor.

Full coverage vital preparation Zirconia crown for the mandibular right canine.

Composite restoration for the mandibular left canine.

To help the patient visualize the final outcome, a diagnostic wax-up was performed, followed by a chairside mock-up using temporaries. This allowed the patient to better understand the proposed changes in tooth form and alignment.

After obtaining the patient’s approval, minimal conservative tooth preparation was carried out for the veneers and crown. The restorations were subsequently fabricated and delivered. The composite restoration for the mandibular left canine was also completed, achieving the desired result.

Accurate Diagnosis is the root to a correct prognosis

An 11-year-old male patient reported to the clinic with a palatal swelling, with no significant history provided initially.

After a thorough clinical evaluation of all teeth, no obvious cause was immediately evident. The patient was started on antibiotics and asked to return after 3 days for reassessment.

At the recall visit, the swelling remained unchanged with absolutely no improvement. This prompted a re-evaluation of all the teeth, including vitality testing.

Interestingly, the upper left lateral incisor tested negative on the cold test.

On further probing, the patient then recalled a fall almost 3 years ago — a detail that had not been mentioned during the first visit.

With this crucial piece of history and clinical findings, the diagnosis of a non-vital tooth with palatal involvement was considered, and root canal treatment was initiated. A closed calcium iodoform dressing was placed, and the patient was scheduled for review after 5 days.

At the 5-day follow-up, the palatal swelling had completely subsided, confirming the odontogenic source of the infection.

Sometimes the smallest detail in history can unlock the entire diagnosis.
