What Is Melasma?
Melasma is a common skin condition that causes brown or grayish-brown patches on the face. Its prevalence ranges from 1.5% to 33% depending on the population. There are several expert-backed treatments for melasma. They include sun protection, hydroquinone, tretinoin and a mild corticosteroid, triple combination cream, chemical peels, microneedling, platelet-rich plasma and tranexamic acid.

1. Sun Protection
Sun protection is an essential part of treating melasma, says Rebecca Marcus, M.D., a Dallas-based board-certified dermatologist and founder of MaeiMD, a company that creates science-backed skincare. Even if you’re using effective topical and oral treatments for melasma, a few minutes of sun exposure can undo all of your hard work, she adds.

For best results, Dr. Marcus recommends a layered approach to sun protection. This includes:
- Mineral-based sunscreen with an SPF of 30 or higher (titanium and zinc oxides in mineral-based sunscreens have been shown to help with melasma by blocking harmful UV rays)
- Using hats and ultraviolet (UV) protective clothing when possible
Staying out of the sun
Dr. Marcus also notes that visible indoor light can trigger melasma, so it’s best to wear sunscreen at all times.

2. Hydroquinone
Hydroquinone is a chemical bleaching agent that has been used for several decades, says Dr. Marcus. It works by blocking an enzyme called tyrosinase, she explains. By blocking this enzyme, the pigment synthesis process is stopped, helping decrease the amount of pigment in the skin and reducing melasma spots, adds Dr. Marcus.

Hydroquinone cream is used as a depigmentation agent. Typically, a thin layer is applied to the entire face one to two times per day for three to six months. It’s important to apply the cream to the entire face to avoid uneven pigmentation. If no changes in pigmentation are seen after two to three months of daily use, hydroquinone should be discontinued. Currently, hydroquinone is not U.S. Food and Drug Administration (FDA) approved for over-the-counter sales and is only available with a prescription.

One side effect of hydroquinone use is irritation, says Dr. Marcus. In rare cases, hydroquinone use may provoke a condition called exogenous ochronosis, which results in a blue-black discoloration of the skin. Limiting the use of hydroquinone to a maximum of three months minimizes the likelihood of developing this rare side effect, she notes.

It’s also important to note that hydroquinone can make skin more sensitive to the sun, so it’s important to avoid excessive sun exposure when using this treatment, says Michele Green, M.D., a board-certified cosmetic dermatologist based in New York City.

3. Tretinoin and a Mild Corticosteroid
The combination of taking tretinoin (a retinoid medication derived from vitamin A) and a mild topical corticosteroid (an anti-inflammatory medication which improves cell turnover and reduces inflammation), says Dr. Marcus.

Tretinoin promotes the loss of pigment by increasing the turnover of skin cells and turning off the biochemical pathway that produces melanin. It’s unclear how a mild corticosteroid alone improves melasma, but when used in combination with a treatment like tretinoin, corticosteroids can create a synergistic effect while reducing irritation caused by the tretinoin.

While corticosteroids can be used in conjunction with tretinoin in the short-term to treat melasma, they should not be used for an extended period, as they can lead to skin thinning, says Dr. Green.
