INTRAMUSCULAR (IM)
INJECTION TECHNIQUE

Intramuscular (IM) injection is a common and important nursing skill used to deliver medication deep into a muscle so it can be absorbed quickly into the bloodstream. Here’s a clear, student-friendly breakdown 👇

💉 Intramuscular (IM) Injection Technique

1. Preparation

√Wash hands properly 🧼
√ Gather equipment:
√ Sterile syringe and needle
√ Medication
√ Alcohol swab
√ Cotton/gauze

Check the 5 rights:
° Right patient
° Right drug
° Right dose
° Right route
° Right time

2. Select the Injection Site

Common IM sites:

• Deltoid muscle (upper arm) – small doses

• Vastus lateralis (thigh) – safe for all ages

• Ventrogluteal (hip) – safest for adults

• Dorsogluteal (buttock) – less preferred (risk of nerve injury)

3. Position the Patient

√ Make the patient comfortable

√ Expose the site

√ Relax the muscle (very important to reduce pain)

4. Clean the Site

° Use an alcohol swab

° Clean in a circular motion (center → outward)

° Allow to dry (do not blow or fan)

5. Injection Procedure

• Hold syringe like a dart

• Stretch the skin (or use Z-track technique if needed)

• Insert needle at 90° angle straight into the muscle

• Aspirate (depending on protocol — many guidelines now say no aspiration for vaccines).

• Inject medication slowly and steadily

6. After Injection

√ Withdraw needle quickly

√ Apply gentle pressure with cotton/gauze

√ Do not massage (especially for some medications)

√ Dispose of needle safely in a sharps container

7. Documentation
• Record:
• Drug name
• Dose
•Site
•Time
• Patient response

THE LACRIMAL APPARATUS

The lacrimal apparatus is an important part of the eye responsible for producing and draining tears. It helps keep the eye moist, clean, and protected from infection.

🔹 What is the Lacrimal Apparatus?

The lacrimal apparatus is a system made up of structures that produce, distribute, and drain tears.

🔹 Components of the Lacrimal Apparatus

1. Lacrimal gland

•Located in the upper outer part of the eye (in the orbit)

• Produces tears (lacrimal fluid.

2. Lacrimal duct

• Small ducts that carry tears from the lacrimal gland to the surface of the eye.

3. Lacrimal puncta

• Tiny openings found at the inner corner of the eyelids

• They collect tears from the eye surface.

4. Lacrimal canaliculi

• Small channels that carry tears from the puncta to the lacrimal sac

5. Lacrimal sac

• A small reservoir located near the nose

•Temporarily stores tears

6. Nasolacrimal duct

• Drains tears from the lacrimal sac into the nasal cavity

•This is why your nose runs when you cry.

🔹 Functions of the Lacrimal Apparatus

√ Keeps the eye moist and lubricated

√ Washes away dust and microorganisms

√ Provides nutrients and oxygen to the cornea

√ Contains antibacterial substances like lysozyme

√ Helps in clear vision by maintaining a smooth optical surface

🔹 Tear Flow

1. Tears are produced by the lacrimal gland

2. Spread across the eye when you blink
Collected by lacrimal puncta

3. Drain through canaliculi → Lacrimal sac

4. Exit via nasolacrimal duct into the nose

🔹 Clinical Importance (For Nursing Students)

• Blocked nasolacrimal duct → causes watery eyes (epiphora)

• Infection of lacrimal sac (dacryocystitis) → swelling and pain near the nose

• Dry eyes may occur if tear production is reduced

WHY NIGHT SHIFT IS MORE DIFFICULT THAN DAY SHIFT IN CLINICAL PRATICES.

Night shift during clinical practice is often more difficult than day shift for several reasons. As a student nurse, you may notice these challenges in the hospital.

1. Lack of Sleep and Fatigue 😴
At night, the body naturally wants to sleep. Working during this time can make you feel tired, weak, and less alert. Fatigue can make it harder to concentrate on patient care.

2. Reduced Staff
Night shifts usually have fewer nurses and doctors on duty. Because of this, each nurse may have more responsibilities and less help compared to the day shift.

3. Limited Supervision for Students
During the day, there are usually more supervisors, instructors, and senior nurses available to guide student nurses. At night, supervision may be limited.

4. Emergency Situations
Many emergencies can occur at night, and with fewer staff members available, nurses must respond quickly and manage situations effectively.

5. Limited Hospital Services

As a student nurse, it is important to understand evidence-based practice (EBP) because it helps you give safe, effective, and high-quality patient care. Evidence-based practice means using the best research evidence, clinical experience, and patient needs when making decisions about patient care.

DRINKING WATER EVERY MORNING IS VERY IMPORTANT FOR GOOD HEALTH

Some of the importance of drinking water every morning include:

1. Rehydrates the body 💧
After sleeping all night, the body becomes slightly dehydrated. Drinking water in the morning helps replace the lost fluids.

2. Improves digestion 🍽️
Water helps the stomach and intestines start working properly and prepares the body to digest food during the day.

3. Helps remove waste from the body 🚰
Drinking water supports the kidneys and helps flush out toxins through urine and sweat.

4. Boosts energy and brain function 🧠
Water improves blood circulation and helps the brain work better, making you feel more alert.

5. Supports healthy skin ✨
Drinking water regularly helps keep the skin hydrated and healthy.
Helps with bowel movement 🚻
Morning water can help prevent constipation and make it easier to pass stool.

MY FIRST EXPERIENCE AT THE Hospital with A NEWBORN BABY WITH RESPIRATORY DEPRESSION.

My first experience at the hospital caring for a newborn baby with Respiratory Depression was both challenging and educational.

As a student nurse on clinical practice, I had the opportunity to observe and assist the medical team in managing a newborn who had difficulty breathing shortly after birth.

The baby was delivered in the labor ward, but immediately after birth the baby was not crying and the breathing was very slow.

The baby's skin appeared slightly bluish, which showed that the baby was not getting enough oxygen. The midwife and nurses quickly recognized that the newborn was experiencing respiratory depression.

The doctors , nurses and other students immediately began resuscitation procedures according to the principles of Neonatal Resuscitation.

The baby was dried, stimulated gently, and the airway was cleared to ensure there was no mucus blocking breathing. Oxygen was then administered, and the baby's breathing and heart rate were closely monitored.

As a student nurse, I assisted by preparing the equipment, observing the baby's condition, and learning how the nurses responded quickly in such an emergency situation.

It was my first time seeing such a case, and I felt nervous at first. However, watching the experienced nurses work calmly and efficiently helped me understand the importance of teamwork, quick decision-making, and proper newborn care.

After a few minutes of intervention, the baby began to breathe more regularly and started crying, which was a very relieving moment for everyone in the room. The baby was then placed under close observation in the neonatal unit.

This experience taught me many important lessons about newborn care, emergency response, and the management of respiratory problems in infants.

It also increased my confidence and motivated me to continue learning more about neonatal care as a future nurse.
