The winged scapula, or scapula alata, is a condition in which the scapula protrudes beyond the level of the rib cage, appearing “wing-like.” It can be caused by various factors that disrupt the normal function of muscles, nerves, or skeletal structures. Below are the main causes:
1. Nerve Damage
• Damage to the long thoracic nerve (n. thoracicus longus), which innervates the serratus anterior muscle (m. serratus anterior). This is the most common cause of a winged scapula.
• Injury to the brachial plexus, which may result from severe impacts, stretching, or compression of the nerves.
• Damage to the accessory nerve (n. accessorius), which innervates the trapezius muscle.
2. Muscle Weakness or Dysfunction
• Weakness of the serratus anterior muscle, which may result from overuse or insufficient physical activity.
• Weakness of the trapezius muscle or the rhomboid muscles.
• Muscular dystrophy or myopathies (either inherited or acquired).
