Positive aspiration sign in filler!

Every injector seeing this just puckered a little...

See that red in the tip of the syringe? Thats blood. This is known as a positive aspiration sign in dermal filler treatments and is a signal we could be inside a blood vessel.

This particular positive was down on the bone in the lateral cheek which according to protocol should be "free of blood vessels".

However every face is unique, anatomy has variants, and "safe zones" cannot be taken for granted. This patient had a blood vessel in what is normally an avascular zone, and thank goodness we aspirated before injecting the supposedly "safe" zone. This could have resulted in a vascular occlusion had the injection been placed. Which would have needed emergency dissolving to return blood flow.

So how do we handle this? Simple. We dont panic. We remove the needle from the injection site. Hold pressure to minimize any bruising, and reassess our placement. In this case on assessment we decided to use a cannula to place the filler in a different layer of the tissue. Allowing for a beautiful result and a happy client with great perfusion post treatment.
