A week-long Health & Wellness Fair is being conducted in front of the 𝗟𝗮 𝗧𝗿𝗶𝗻𝗶𝗱𝗮𝗱 𝗠𝘂𝗻𝗶𝗰𝗶𝗽𝗮𝗹 𝗧𝗼𝘂𝗿𝗶𝘀𝗺 𝗕𝘂𝗶𝗹𝗱𝗶𝗻𝗴 𝗼𝗿 𝗮𝘁 𝘁𝗵𝗲 𝗠𝘂𝗻𝗶𝗰𝗶𝗽𝗮𝗹 𝗣𝗮𝗿𝗸.

1. Primary Care Facility Empanelment
2. Random Blood Sugar, Blood Uric Acid, Cholesterol
3. Eye Screening
4. Breast Screening
5. Family Planning Services
6. Cervical Cancer Screening through Visual Inspection with Acetic Acid (VIA)
7. Dental Checkup
8. Blood Donation
9. Chest X-ray—February 10-11, 2026 ONLY
10. HIV Screening
11. Nutrition Counseling

1. You may fill in your names in our schedule for your preferred date and time, depending on its availability. Do take a picture or take note of your schedule as a personal reminder
2. Clients can only avail 𝗢𝗡𝗘 (𝟭) 𝘁𝗿𝗲𝗮𝘁𝗺𝗲𝗻𝘁 𝗽𝗲𝗿 𝘄𝗲𝗲𝗸.
3. 𝗢𝗡𝗘 (𝟭) 𝘀𝗹𝗼𝘁 is equivalent to only 𝗢𝗡𝗘 (𝟭) 𝗰𝗹𝗶𝗲𝗻𝘁.
4. Rescheduling is 𝗛𝗜𝗚𝗛𝗟𝗬 𝗗𝗜𝗦𝗖𝗢𝗨𝗥𝗔𝗚𝗘𝗗. Patients who come beyond their allotted time, even with a reserved slot, 𝗪𝗜𝗟𝗟 𝗡𝗢𝗧 be accommodated with respect to other scheduled patients.
5. Posting of monthly slots will be available every 𝗟𝗔𝗦𝗧 𝗪𝗘𝗘𝗞 𝗢𝗙 𝗧𝗛𝗘 𝗠𝗢𝗡𝗧𝗛.

3.Provide information regarding your medical condition and any medication you are taking.
