What is galactorrhea?
Galactorrhea (guh-lack-toe-REE-uh) happens when your breastsunexpectedly produce milk or a milk-like discharge. This ni**le discharge may leak from your breast on its own or when touched. It’s not related to milk production in breastfeeding(chestfeeding) or pregnancy. Galactorrhea sometimes indicates an underlying health condition, but is most often caused by too much prolactin. Prolactin is a hormone that triggers milk production. It’s made by your pituitary gland, a gland at the base of your brain.

Symptoms and Causes
What is the most common cause of galactorrhea?
The most common cause of galactorrhea is a benign (not cancerous) tumor on your pituitary gland. The tumor causes your pituitary gland to make too much prolactin (hyperprolactinemia). Prolactin is the hormone responsible for milk production after you give birth. Excess prolactin tricks your body into thinking it should lactate (or make milk), which is why you leak milk from your ni**les.
Other causes include:
Taking medications including birth control pills, blood pressure medicine or antidepressants.
Overstimulating your breasts (through s*xual activity, clothing or frequent breast self-exams).
Thyroid disorders.
Chronic kidney disease.
Taking herbal supplements like fenugreek or fennel seed.
Using opioids.
Injury or trauma to your spinal cord.
What are the symptoms of galactorrhea?
The main symptom of galactorrhea is leaking a light white, milk-like discharge when you’re not breastfeeding or pregnant. It typically affects both breasts and may flow if you squeeze your ni**le or leak on its own.
Other symptoms associated with galactorrhea may include:
Amenorrhea (infrequent or stopped menstrual periods).
Vaginal dryness.
Headache.
Reduced s*x drive.
New hair growing on your chest or chin.
Acne.
Erectile dysfunction.

Menstrual disorders
These are normally divided into dysmenorrhoea, menorrhagia/metrorrhagia and amenorrhoea. Dysmenorrhoea is normally a clinical term applied to painful periods. This pain can be so excruciating that your normal everyday activities might get hampered immensely. Menstrual flow that continues for more than seven days is considered menorrhagia or excessive, heavy bleeding. On the other hand, metrorrhagia is bleeding between periods. Absence of periods is called amenorrhoea. All these conditions call for extensive medical examination - check for fibroids or polyps or hormonal imbalance. Hormonal checks need to be done to pinpoint the exact causes. Some conditions can be treated with medication and lifestyle changes.
Red alert
Different gynaecological conditions have different symptoms. Doctors say that if fatigue persists, one should be getting a wake-up call. "Tiredness, lack of energy, breathlessness and palpitation can be due to anaemia probably related to heavy menstrual bleeding," says Dr Zoha. "Poor appetite and weight loss, on the other hand, could be due to underlying ovarian cancer. Menstrual irregularity can be a presenting symptom of polycystic ovarian disease or underactive thyroid gland (Hypothyroidism)."

Fibroids
Experts say fibroids are, broadly speaking, benign tumours. But if they grow to gigantic sizes, they can pose problems for pregnant women. So, what are the best options to treat them? "Fibroids can be treated either by removal of the tumours keeping the uterus intact (Myomectomy) or by removal of uterus along with the tumour (Hysterectomy)," says Dr Zoha. "Myomectomy can be done either by traditional open surgery or modern microsurgical technique. Depending on the location of fibroids the microsurgical technique can be through natural birth passage (Hysteroscopic Myomectomy) or the small holes on the tummy (Laparoscopic Myomectomy). The laparoscopic/ Hysteroscopic surgery has got some definite advantage over open surgery. It is associated with smaller abdominal scar, lesser blood loss and lesser post-operative pain, briefer hospital stay amd quicker recovery." However, he adds that so far, there is no safe and permanent medicinal cure available for fibroids.
Ovarian cyst
Doctors say that an ovarian cyst is, generally speaking, a sac that appears on a woman's o***y and it's normally filled with fluid. Some ovarian cysts can disappear without any treatment. There can be a cyst because of an abnormal cell growth, but this kind of cyst is not too common, experts say. Symptoms might include pelvic pain, a frequent need to urinate, heavy periods and bloating.

Pelvic inflammatory disease (PID)
Again, this is a kind of infection of the pelvic area. This can occur after pregnancy or following some gynaecological procedures. The common symptoms include abnormal va**nal discharge, pain during in*******se and erratic menstrual bleeding. Sometimes it can asymptomatic, so you need to be extra careful as PID raises infertility risks. Mostly, antibiotics are recommended to contain PID. Safer s*xual practices are also advised

Chronic pelvic pain
This can be a manifestation of other medical conditions or it can be a condition in itself. In any case, experts say that this is one of the most common complaints from women. Needless to say, you can experience severe and steady pain or intermittent pain or cramps in the pelvic region. You can also feel some heaviness within your pelvis. Other concomitant symptoms include sharp pain during s*x or while urinating. Do not self-diagnose or take self-prescribed drugs. See a doctor if the pain persists more than it should. "Pelvic and lower abdominal pain during menstruation can also be an early indication of pelvic endometriosis," says Dr Zoha.

Sexually active women must remember to wash their va**nas after in*******se. And they must absolutely ensure that their male partners use condoms to prevent s*xually transmitted diseases including HPV infection. Medical experts say most Indian teenagers are s*xually active and they opt for multiple partners. "Often they are not aware of contraceptives and are vulnerable to STDs. Sometimes s*xually transmitted diseases would present themselves through va**nal discharge," says Dr Datta.
Corollary: When we speak of HPV infection, we need to caution women against cervical cancer, which, according to oncologists, mostly occurs in women infected with the Human Papilloma Virus (HPV) through s*xual activity. "Cervical cancer has a long pre-cancerous stage and can be easily diagnosed and treated by doing a Pap test of cervical smears. All women should start this test at the age of 21 and repeat it every three years till 29; every five years between age 30 and 65 (and a test for HPV where available), and thereafter only if some abnormality gets detected in previous tests," says Dr Arunabha Sengupta, consultant surgical oncologist, Kolkata. "Pap tests diagnose only cervical cancer, not ovarian or uterine body cancer. Western countries recommend cervical cancer vaccine (available in India) against for all girls by the age of 14. Early stage cervical cancer is one hundred per cent curable."

Improvement in personal hygiene and safe and responsible s*xual behaviour are two of the most important measures to prevent these kinds of problems. In fact, experts say awareness about va**nal health should start from childhood. Often young girls would contract various kinds of va**nal infection, mainly because of poor hygiene or a lack of awareness about it. Cleaning with just a plain soap and water would do. Once a girl reaches puberty, a kind of self-cleaning mechanism sets in. "When a girl gets her period, oestrogen comes from the ovaries and it helps in the self-cleaning process by producing lactic acid," says Dr Gupta.
Mature women who have undergone menopause would require a different regimen for their va**nal health. "This is because around this age, oestrogen gets dried up, va**nal skin gets thinner and becomes prone to infection and bruising during in*******se because of a lack of lubrication," says Dr Gupta. "So this is a case of va**nal dryness along with va**nal irritation. You should consult a doctor. Often a local application of oestrogen cream is prescribed, depending on individual cases."

Another common infection is bacterial vaginosis - it is more of an inflammation though. "This is not really a s*xually transmitted infection, but may occur because of low hygiene or low immunity," says Dr Jayanta Gupta, senior consultant (gynaecology and obstetrics), Kolkata. "In this case, the local bacterial flora in the va**na is changed. Normally the flora contains lactobacilli or good bacteria which helps maintain the pH balance (how acidic the va**na is). Sometimes, because of heavy medication or anti-biotic doses, the lactobacilli-producing bacteria get killed and the other less benevolent forms of bacteria start colonising the va**na." For bacterial vaginosis, one should seek medical attention whereby an anti-biotic course would be prescribed.
Trichomoniasis is another infection caused by a parasite called T Vaginalis. This infection is s*xually transmitted. "Whenever a woman contracts this, her partner should also be treated because it's coming from him," says Dr Gupta. "Otherwise there could be a relapse."
