Repost Dr. Edward Boshnick: This is a photo of a 20.8 mm diameter scleral lens over an eye that underwent a corneal transplant operation over 25 years ago. This cornea is extremely profound to the extent that no conventional contact lens or scleral lens was able to properly fit this eye and provide this patient with clear vision. For over 25 years this patient's visual acuity with this eye was less than 20/1000. To properly vault the front surface of this eye we had to fit this eye with a 20.8 diameter scleral lens. In this photo the arrow in the upper left corner is pointing to the edge of the scleral lens. The arrows in the center are pointing to the host-donor interface of the transplanted cornea. This patient's corrected visual acuity with the scleral lens is 20/20 with comfortable all day lens wear.

One option to solver dry eye is the use of scleral lenses. Though commonly used to treat corneal irregularities and refractive errors, scleral lenses can also provide great relief to dry eye patients. Here are six reasons why.

1) They don’t irritate the cornea as other contacts do:
Standard soft lenses sit on the cornea, which can be extremely irritating for dry eye patients. Scleral lenses, on the other hand, sit on the sclera (the white of the eye) and vault over the cornea. The lenses don’t touch the corneal
surface at all, thereby minimizing irritation.

2) Their design ensures constant hydration of the eye:
The space between the surface of the cornea and the scleral lens is filled with a saline solution. This solution acts as a constant source of hydration for the eye, providing unbeatable relief for dry eye patients. Artificial tears as well as antibiotic can be added into the bowl of the lens prior to insertion to assist in lubricating and promote healing of the ocular surface.

3) They protect the cornea:
Dry eye syndrome leaves the eyes more sensitive and the corneas more vulnerable to injury. Scleral lenses can act as a shield between a patient’s eyes and their eyelids/the outside environment. This shield protects the eyes from further irritation or damage.

4) They allow the eye to regain a healthier appearance:
By giving the eyes a shield from the outside world and a constant hydration source, scleral lenses play a therapeutic role in helping the eye heal or stay healthy. In turn, the eyes retain a healthier appearance.

5) Patients can continue using eyedrops and artificial tears during lens wear:
There is no “either-or” proposition between scleral lenses or eyedrops. With that said, many patients may find that they can decrease the frequency of artificial tear use once they start wearing scleral lenses, or that they only need eyedrops at night after they have removed their lenses for the day.

6) Scleral lenses can drastically improve quality of life:
Scleral lenses decrease pain and/or discomfort, eye redness, the need to repeatedly scratch the eyes or apply eyedrops, and simultaneously provide
crisp, clear vision during wear.

This course will review how technologies contribute to increased scleral lens fitting success. Drs. Barnett and Fadel will share how to:

* Determine better initial lens selection
* Reduce the number of lenses applied on the eye
* Allow for lens customization
* Incorporate more accurate lens assessment with fewer lens reorders
* Reduce chair time, costs and frustration for both the practitioner and patient

This course will utilize current scleral lens terminology to cover the latest information on scleral lens fitting and assessment with different techniques. An ocular assessment of scleral lens fitting will be reviewed.
