Cleft rhinoplasties are some of the most difficult cases given the amount of asymmetry that often underlies the structure of the nose. In addition, thick skin can make it difficult to increase projection while also creating symmetry due to the lack of stretch of the skin.
The best cartilage for these instances is autologous rib cartilage. It allows for a sufficient amount and sufficient strength of cartilage for grafting. I made a small 1 cm incision in a hidden incision underneath the breast and and was able to obtain a 4.5 cm piece of rib cartilage. This cartilage was carved and placed to open her nasal valves and straighten the deviation that she had.
She had a lack of support at the tip which I supported and lifted. The improved symmetry and tip refinement can be seen from the overhead view.
At the same time, I did a revision on her previous cleft lip scar to help reapproximate the muscle and allow for a more symmetric upper lip which allowing the upper lip contour to be less noticeable.

Intraoperative photography from multiple views is always important to evaluate the symmetry, angles and details of every rhinoplasty. For this patient, I did a rhinoplasty with rib cartilage harvest.
I made a small 1 cm incision in a hidden incision on the chest and was able to obtain a 5 cm piece of rib cartilage. This was used for structural grafting to improve her nasal valves and to straighten the deviation that she had.
Her profile was heavier with a downward pointing tip and lack of support, causing drooping. I supported and lifted the tip while refining it. I also redid her osteotomies which is when the bones are surgically and meticulously fractured to achieve improved symmetry.
The improved symmetry and tip refinement can be seen from both the overhead view as well as the base view.

This patient is almost two years out after a septorhinoplasty! I love how gentle her profile and oblique views are now, which has eliminated the heaviness of her nose.
I also reduced the flare of her nostrils ever so slightly with weir incisions. Many times patients and surgeons shy away from these incisions as they are worried about scarring. However, when carefully planned and executed, weir incisions are hidden and heal to become imperceptible.
I’m happy that we were able to accomplish her aesthetic goals while at the same time improving her breathing. Can you see the improvement in symmetry of her nostrils?

I really enjoy periorbital rejuvenation as it can greatly enhance the entire brightness of a patient’s face. In this patient, an upper lid blepharoplasty removed excess skin while preserving her natural crease and eye shape. Her own eyelid creases were utilized for the placement of the incisions, keeping them subtle and hidden.
She also had a transconjunctical lower lid blepharoplasty with fat repositioning in order to smooth the appearance of her eye bags, allowing for a more refreshed appearance. The infraorbifal fat was kept intact and moved into the hollow tear troughs, allowing the fat to act as a natural filler.
I’m so excited for results and so happy that we were able to give her a fresh new beginning to 2024!
