Clear aligners may be easier to clean than fixed appliances, but attachments still create plaque-retentive areas where demineralization can occur. A 2025 BMC Oral Health study followed 40 clear-aligner patients for six months. All brushed with 1,450 ppm fluoride toothpaste. Additional treatment groups received CPP-ACP, fluoride varnish, or an intensive nano-hydroxyapatite protocol using daily 1% nHAp gel plus professional 5% applications.
DIAGNOdent values worsened by 4.35 units with toothpaste alone, but improved by 4.02 with CPP-ACP, 4.45 with fluoride varnish, and 5.36 with nano-hydroxyapatite.
At first glance, nHAp looks like the winner. But the study was small, nonrandomized, and the treatment regimens were not equivalent. The nHAp group received daily home therapy plus three professional applications, while fluoride varnish was applied only twice.
The difference between nHAp and fluoride varnish was just 0.91 DIAGNOdent units. That was statistically significant, but there is no established clinically meaningful threshold for such a small change.
DIAGNOdent is also not a direct mineral meter. It measures laser-induced fluorescence and is best used as an adjunct to clinical examination. Research shows it is useful for detecting early lesions, but less precise for quantifying small changes in actual mineral content.
The broader evidence is encouraging but still limited. Nano-hydroxyapatite appears biologically active and promising, especially as an adjunct. CPP-ACP also has supportive but inconsistent evidence. Fluoride remains the benchmark because it has the deepest clinical evidence base.
The practical takeaway is not to pick a winner. It is to build a better prevention workflow. Assess white spot lesion risk before orthodontics. Photograph suspicious areas. Reinforce plaque control and diet. Make sure aligners go back over clean teeth. Monitor attachment margins closely. If early lesions appear, control the disease environment first, then consider adjunctive remineralization.
