A recent consumer report found that 80% of people feel positively about aesthetic treatments, but one-third of those who've had something done wish they hadn't. That gap tells a story, and I think it's an important one.

In my experience, regret in aesthetics almost never comes from the treatment itself. It comes from a mismatch between what was promised and what was delivered. It comes from a provider who said yes when they should have said 'not yet' or 'not that.' It comes from a consultation that felt more like a sales pitch than a conversation.

This is exactly why I built Astra the way I did.

If you’re on a GLP-1 medication and you’ve noticed changes in your face, you’re not imagining it. Here’s the anatomy of what’s happening, and why the solution isn’t just ‘more filler.’

GLP-1 weight management medications work by suppressing appetite and accelerating fat metabolism. That’s the goal. But fat loss doesn’t happen selectively. It happens systemically, including in the face. The buccal fat pad, the temporal fat compartments, the periorbital fat — these all contribute to the structural support of your facial features. When they reduce rapidly, the overlying skin can’t always keep pace, which leads to hollowing, laxity, and a gauntness that patients often describe as looking older or more tired than they feel.

This is a structural problem, and it requires a structural solution.

That doesn’t automatically mean filler. In fact, reaching for volume replacement as a first response can sometimes make things worse, adding bulk without addressing the underlying tissue quality. What I look at first is skin laxity, collagen density, and the specific compartments that have been affected. From there, a thoughtful plan might include biostimulatory injectables to rebuild collagen, energy-based treatments to address laxity, and strategic RHA filler only where it’s genuinely needed.

The goal is always to restore what was lost, not to create something new.
