65% of full-thickness rotator cuff tears found on imaging are completely asymptomatic — no pain, no loss of function. A tear on a scan is a finding, not a sentence.
Research consistently shows that structured physical therapy produces outcomes comparable to surgery at 12–24 months for small and medium tears. And 75% of patients with atraumatic full-thickness tears avoided surgery altogether after completing a structured PT program.
That doesn’t mean surgery is never right — but it should never be the default.
