I recently pushed my adapalene and chemical exfoliant too hard. Now everything I put on my skin burns - even moisturizer. Oops 😅

How do I get back to normal? Nothing but cleansing with gleanser and slathering with cicalfate every night until better!

After a few nights, my skin starts behaving like itself again and I can get back to my actives slowly.

Some of my other favorite occlusives to do this with are healing balm, cicaplast, and good ol’ petroleum jelly!

CASE STUDY 📓🔬

Client in mid 40’s comes to me complaining of irregular skin texture. Has a past history of taking Accutane (isotretioin) in their youth. Does not have an established routine.

Skin analysis with bare hands allowed me to appreciate her rough texture. Her skin was easy to sting or burn with any acids applied. She also seemed to have open and closed comedones, and non acne like pustules.

Here’s how I managed this:

I gave her products to strengthen her barrier:

- The gleanser to cleanse with as it contains 50 percent glycerin (a humectant) to hydrate the skin while cleansing. Her skin barrier seemed in need of all the help it could get. This option was perfect to get her clean while maintaining her lipid barrier.

- APAD azelaic derivative serum to apply twice daily - this serum can provide similar benefits to a traditional azelaic serum without being quite as strong on her weakened barrier. Azelaic acid is known for reducing the bacterial load on the surface of the skin as well as down regulating inflammation.

- CREAM was also given as it has a high concentration of ceramides to help restore her skin barrier. This was a crucial step in minimizing texture.

Once her skin barrier was happier and less easily disrupted, it was time to add in gel. This gentle retinoid applied 3x weekly helped clear up acne and residual texture.

Once her skin acclimated, we moved her to a more traditional azelaic acid product from that uses a modest 10% concentration in a fully solubilized water based serum.

These simple changes to her routine + the use of a daily sunscreen and several light chemical peels transformed her skin texture.

Remember that when you are treating acne, if the skin barrier is suffering for any reason, you must address it FIRST before adding in more aggressive active ingredients!

You wanna dermaplane at home before a big event? Or you just like how it feels to remove your peach fuzz? No big deal if you like to do it home! And NO you aren’t causing ‘microtears’ 🤪

It’s perfectly safe to remove your hair with a clean serrated blade at home. The only difference of someone else doing it for you is perhaps more attention to detail, greater exfoliation with a scalpel, and a potentially smoother result.

However, there are instances where I don’t recommend a client dermaplane at all. These special exceptions are those that are extremely acne prone, sensitive, or have existing barrier disruption. Dermaplaning can sometimes aggravate these conditions and it’s best to patch test areas before you commit to the entire face.
