CUPPING THERAPY ON THE GOVERNOR VESSEL (DU MAI) – EFFICACY, RISKS & CONTRAINDICATIONS
In response to Trương Lan's question: "Is it permissible to perform cupping on the Governor Vessel?" This is a common and practical inquiry. Within the field of Traditional Chinese Medicine (TCM) healthcare, there are various perspectives regarding this practice. Currently, there are no official TCM documents (standardized academic curricula or hospital guidelines) that strictly prohibit cupping on the Governor Vessel.
Let’s explore this in more detail.
I. Key Concepts
Cupping Therapy: A method using suction cups placed on the skin to create negative pressure. This pulls the skin and superficial blood vessels upward, causing localized congestion, which stimulates blood circulation, relieves pain, reduces inflammation, and aids detoxification.
Governor Vessel (Du Mai): Originates in the Kidneys and descends to the perineum (Huiyin). From the perineum, it ascends along the spinal column to the Fengfu point, where it branches—internally entering the brain and externally following the midline of the head, passing through the Baihui point to the Renzhong and Yinjiao points (upper and lower surfaces of the upper lip).
As the "Sea of Yang Meridians," the Governor Vessel plays a vital role in the body, directly connecting with the Kidneys and the Brain while running along the spine. Consequently, any therapeutic technique involving the Governor Vessel must be performed with extreme caution.
II. Cupping on the Governor Vessel
Cupping along the Governor Vessel is applied in the following scenarios:
Cupping Diagnosis (Gua Zhen): Placing cups according to a diagnostic map along the Governor Vessel. By observing skin reactions and marks during and after the session, practitioners can evaluate the health status of internal Zang-Fu organs.
Note: Retention time should generally not exceed 10 minutes. Vacuum suction cups are preferred to avoid thermal interference and minimize skin damage.
Pain Management: For conditions like neck, shoulder, or lower back pain, specific points on the Governor Vessel are used, most commonly the Dazhui point.
Additionally, Governor Vessel points are utilized in protocols for treating the common cold, tonifying the Kidneys, or managing bronchitis.
III. Critical Precautions
While cupping is effective, it requires precision, especially when applied to the spine.
1. General Contraindications
Medical Conditions: Strictly avoid for individuals with heart failure, kidney failure, diabetes, high blood pressure, deep vein thrombosis (DVT), or metastatic cancer.
Acute Symptoms: Do not perform on those with fever or convulsions.
Blood Disorders: Contraindicated for hemophilia, those on anticoagulants, leukemia, or low platelet counts.
Systemic Issues: Not for patients with systemic infections, HIV, epilepsy, or severe neurasthenia.
Physical State: Avoid when the patient is intoxicated, excessively hungry, or overly full.
Demographics: Not recommended for young children or pregnant women.
Localized Areas: Do not cup over broken skin, mucous membranes, recent surgical sites, major blood vessels (e.g., carotid artery), nerve plexuses, or lymph nodes.
2. Specific Precautions for the Governor Vessel
Body Composition: Avoid for extremely thin individuals with minimal subcutaneous fat.
Duration: Retention time must not exceed 10 minutes.
Neurological Concerns: Strictly prohibited for patients with brain or neurological pathologies.
Technique: * Use low suction pressure (1 to 1.5 bars depending on tissue thickness).
Do not perform moving/sliding cupping on the Governor Vessel to avoid injury to the spinal processes (bone spurs).
Wet cupping (bloodletting) is not recommended on this meridian.
